INSTRUCTIONS FOR ENROLLING IN OUR STUDY
We are continuing to enroll subjects into this research study. We are interested in individuals with focal segmental glomerulosclerosis (FSGS), nephrotic syndrome, and/or proteinuria who have other family members with kidney dysfunction as well. This study is an effort to identify genes underlying FSGS. It is not a treatment trial and subjects will likely not benefit directly from enrollment in the study. Our study involves obtaining information about a subject's medical and family history as well as a blood and urine sample.
1. Ideally, we like to obtain 2 yellow or green top tubes from adult subjects; one tube from children 12 and under; and one small pediatric size tube from very small children. In small children, blood should be obtained only at a time when blood would be obtained for regular clinical care. On occasion we may request a third tube.

3. Have each adult participant sign the consent form. Older children can give assent, but parents must consent on behalf of each child that participates.

5. Please also send us a copy of the kidney biopsy report from the affected individuals.
